Getting a "Diagnosis Out of Sync" Message
The full message reads: "It appears that the diagnosis for [patient name] has been changed in another note since you started your note. [Name of User] compiled a [specific note type] on [date and time] with a diagnosis of..."

You have three options to reconcile this message:
- Keep = Ignore the conflict and compile the note with the diagnosis as you set it, but do not overwrite the chart face diagnosis.
- Replace = Replace the diagnosis in your note with the above (noted) diagnosis and then review the changes.
- Review = Stop the compilation process and look over your diagnosis.

To avoid this, when you are making changes to diagnoses on existing notes, start with the oldest note you plan to change, chronologically, and work forward. This will avoid the system detecting a conflict between notes, and clinically, this will ensure that your notes are standardized and accurate.
