How to Enter a CLIA Number on a Claim

1. To enter a CLIA (Clinical Laboratory Improvement Amendments) number for a clinician, click on the Settings + Directories drawer from the Chart Room filing cabinets on the right.

2. As a group administrator, find the clinician's name and click on it.

3. The CLIA # field will display so that you can enter the number for that clinician. 


When one of a number of substance abuse-related CPT codes is entered as the first code in the billing list for an encounter, the CLIA # will pull to that claim (box 23 in the HCFA/CSM 1500 and ANSI 837 Loop 2300 REF02).
