This knowledge base article will demonstrate how to enter an ICD-10 diagnosis. Diagnosis fields are present in most notes.

1. From any diagnosis fields, click the Enter button. 

2. There are three ways that can be used to enter ICD10 codes: 

• Pull up using dropdown menus
• Pull up by codes
• Pull up using ICD10 Short List

3. Once you have entered all of the diagnoses, click the Done button. 

4. Next, decide if you want to include the diagnosis in the note. 

• Yes - the entire diagnoses will show including the Axis II-V.
• Omit All - no diagnoses will show in the note.
• Omit Axes II to V - only the diagnosis shown in the field will show.

5. You are able to enter Axis II - V by clicking the  button. Fill in the Diagnosis Details.
