Patient Address List Exported to Excel

If you need to generate, print, or export a list of patients, their DOBs, addresses, email addresses, or phone numbers follow these instructions:


1. Use the "Clinical/Demographic Search" to retrieve the list of patients. This report can be accessed from the Reports dropdown menu on the top-left of your screen.

 


2. In the report, enter your desired search criteria to select the list of patients. For example, if you want a list of all active patients, simply leave "active" checked over on the far left side. If you want a list of patients assigned to a current physician, you can search for patients by specific clinician in the "Primary Clinician" field.

 

3. After you have addressed all desired fields, click the blue  button at the top.

 

4. A list will generate based on the criteria you entered. 

  

5. From here, you can either choose the  option to print your list, or you can choose  and the system will send your report to the Upload Site to be downloaded. Learn more about the Export Process.

 

6. If you choose the "Export" option, a popup window will populate. You will be able to choose what information you want on the report. 

 

7. Select the fields you want populated on your report. Once finished with your selection, click the Export button.

 


8. You will receive a popup explaining that your report will be found on the Upload Site.
