If you do not have the Premium Patient Portal, send a copy of the informed consent to your patient/client by email and ask them to complete and return to you (via scan, fax, or mail). Once received, if in electronic form you can upload to the Upload Site to be on the patient's chart. If received by mail, scan into your computer and then upload to the Upload Site.
